DCHC Earns National Recognition for Excellence in Stroke Recovery Care

We’re proud to announce that Davis County Hospital & Clinics has received a Certificate of Achievement from the American Heart Association for our commitment to improving stroke rehabilitation through the Mission: Lifeline® Stroke Post-Acute Care Initiative. This recognition reflects our dedication to providing exceptional, evidence-based care that supports stroke survivors through every step of their recovery journey.

Raising the Standard for Stroke Recovery

Stroke remains one of the leading causes of serious long-term disability in the United States. With nearly 800,000 Americans experiencing a stroke each year, the importance of high-quality, consistent rehabilitation services cannot be overstated. That’s why we are proud to participate in a program that prioritizes standardized, science-backed practices to improve outcomes for stroke patients.

Through the AHA’s post-acute care initiative, we’ve committed to:

  • Establishing a stroke-specific rehab program

  • Educating our staff on the latest in stroke care and recovery

  • Implementing evidence-based protocols and policies

  • Engaging in continuous quality improvement efforts

What This Means for Our Community

By following the American Heart Association’s 2016 Guidelines for Adult Stroke Rehabilitation and Recovery, we ensure that our patients benefit from proven strategies that aid healing, reduce the risk of complications, and support long-term wellness. Participation in this initiative not only enhances our internal processes but also helps patients and families feel confident in the care they receive during one of life’s most vulnerable moments.

"Participation in this program benefits the patient and the facility by standardizing care practices," said Madeline Pawloski, AHA Post-Acute Stroke Program Consultant. "When there are evidence-based processes during every phase of care, patients have the best opportunity for positive outcomes."

A Team Effort

We couldn’t achieve this without the passion and dedication of our healthcare teams, who go above and beyond to put evidence-based knowledge into practice.

"Davis County Hospital & Clinics is committed to improving patient care by adhering to the latest guidelines," said Tara Porter, RN, Manager of ED/Acute Care. "The post-acute care initiative makes it easier for our teams to apply proven strategies daily, helping our patients recover better and live longer, healthier lives."

Building a Stronger System of Stroke Care

Launched in Iowa in 2022 with support from the Leona M. and Harry B. Helmsley Charitable Trust, the Mission: Lifeline Stroke initiative brings together hospitals, EMS teams, rehab centers, and other care providers to build a coordinated system of stroke care. Through participation, Davis County Hospital & Clinics joins a statewide effort to enhance collaboration and raise the bar for stroke treatment across the continuum of care.

Continuing Our Commitment

This recognition is more than just a certificate—it’s a testament to our ongoing mission to provide high-quality, patient-centered care for the people of Davis County and beyond. We're honored to be part of an initiative that empowers both patients and providers through education, coordination, and compassion.
