At Davis County Hospital & Clinics, we want to make scheduling an appointment easy and convenient. You can request an appointment using the two options below.
Online
To request an appointment online, visit our Patient Portal. From there, click “Request an Appointment” and enter your preferred time. You'll then receive a secure electronic message or phone call to confirm your appointment.
Phone Call
To request an appointment by phone, call 641-664-2145. Requests made during non-business hours, weekends, or holidays will be returned the first business day following the request.
