Community Health Assessment & Health Improvement Plan

Through our CHNAs, Davis County Hospital & Clinics has identified the greatest health needs among our community, enabling Davis County Hospital & Clinics to ensure our resources are appropriately directed toward outreach, prevention, education and wellness opportunities where the greatest impact can be realized.

This year, Davis County Hospital & Clinics is teaming up with Davis County Public Health to gather data for an updated 2024 Community Health Needs Assessment (CHNA). The goal of this assessment is to evaluate current health delivery systems and community health needs and gather updated information regarding future health needs within the community. 

The Community Health Assessment and Implementation Plan was approved by the Board of Trustees on 1-20-2025.
